Independent investigation into Gloucestershire Constabulary following fatal Forest of Dean collision

The Independent Police Complaints Commission (IPCC) has launched an independent investigation into the circumstances of a fatal road traffic incident in Mile End, in the Forest of Dean.

At around 1 am on Sunday 17 April, a Nissan Primera came to the attention of police officers and was later involved in a collision. A passenger in the vehicle, Lucy Vaughan, 22, died in hospital later that morning as a result of her injuries.

The IPCC will examine the actions taken by Gloucestershire officers after they saw the Nissan, as well as looking into the care that officers provided to Miss Vaughan after the collision. Two other police forces, Avon and Somerset Constabulary and Wiltshire Police, are involved in investigating the collision itself. 

IPCC Associate Commissioner Guido Liguori said:

“This is a tragic loss of life and my thoughts are with Lucy’s family and friends. We are now working to fully establish the circumstances leading up to the collision, and what actions the police officers and staff involved took.”
