Response to independent review of Cardiff homicide

The Welsh Government commissioned Healthcare
Inspectorate Wales (HIW) to look into the care and treatment provided to
Nathaniel John, and whether his care was a factor in the death of Stephen Rees
in Roath, Cardiff, in March 2011
The
Welsh Government commissions an independent investigation following every
homicide, if the perpetrator has been in contact with mental health services in
Wales within the last 12 months.
Chief Medical Officer for Wales, Dr Ruth Hussey,
said:
“My thoughts are very much with the families of
those affected by this extremely serious and sad case.
“While risk can never be eliminated entirely, it
is vital we do everything possible to identify any shortcomings in care and put
in place measures to reduce the risk of such incidents happening again.
“In this specific case, there were several key
findings of concern. The Welsh Government expects the local health board
involved to work with other agencies to take the appropriate actions and learn
from this tragic case.
“Local organisations have action plans in place to
address the recommendations in the report, and this will be followed up with
them by the Welsh Government.”
The
report makes six recommendations in respect of the care and treatment provided
to Nathaniel John.
