Welcome to Great Minds Together

Great Minds Together support children and young people with special educational needs and social, emotional and mental health needs as well as their families and the professional networks around them. The age range of the young people we can support is 0 - 25 years old. There is no age range for support to their families. 

Our organisation exists to prevent one of the three following elements: 

  • Wrongful hospitalisation
  • Unnecessary Police Intervention
  • Where possible, prevention of children entering the care system by supporting families in an intensive way to ensure positive outcomes 

We are a not for profit organisation that aims to deliver better outcomes for children and young people, particularly those with social, emotional and mental health needs and/or special educational needs and disabilities. 

Our Tier 3.5 and Outreach services are formally supported by NHS England.

Outreach services

Our Outreach service is a free service for families that need support navigating the complex SEND, Mental Health, Education, Housing and Social Care systems. Our team deliver 1:1 intervention’s, bespoke to each individual young person whilst also providing crucial wraparound support for their families

Tier 3.5 Therapeutic Intervention Programme

The Great Minds Together Tier 3.5 Programme is a specialist therapeutic intervention specifically delivered to prevent wrongful hospitalisation and unnecessary police intervention / wrongful criminalisation for young people with additional needs  (including tier 4 step down).

Autism Central

Great Minds Together are able to offer family members, carers and personal assistants of autistic children and adults a variety of group and individual sessions. These will focus on meeting the needs of autistic people and on signposting to local and national services.

Ethos, Aims and Objectives

Our ethos encompasses providing individually tailored therapeutic intervention plans that are bespoke to the needs of the young people and their families and which are reviewed and updated on a regular basis, in partnership with our young people and families. 

 

NHS Autism Peer Education programme

Great Minds Together & BILD were appointed by NHS - Health Education England to deliver the first ever Autism Peer Education pilot programme. As a result of the positive outcomes the Government allocated the first ever budget to develop a National Autism Peer Education delivery programme.

First National Autism Peer Education Model

"Health Education England commissioned Great Minds Together to design & deliver a peer education project to support the needs of families, personal assistants and small independent care providers throughout the Covid pandemic, and to provide learning and resources to upskill and reduce the potential for unnecessary admissions for autistic people into hospital. 

The project is the first ever Peer Education Programme that treats the family as the workforce and uses an innovative and responsive model to rapidly provide bespoke education to focus on optimal mental health support.   

It is vital that the importance of families is acknowledged when recognising the essential role that they play in providing care and support. 

The flexibility of the Great Minds Together model of delivery, results in families being educated, guided, and supported to ensure they are armed with essential knowledge and skills to keep their loved ones safe, as well as education to develop strategies that reduce risk of crisis. 

Families feel empowered & supported, leading to a reduction in the use of health & care services that are already under pressure throughout the system. The model offers tremendous potential to positively impact on the quality of life, not only of the autistic person but also on their families and carers, and crucially is available flexibly and on demand.
